Sebos-4StarAt Sebo’s Nursing & Rehabilitation Center, we understand that the transition to a skilled nursing center, whether it be for short-term rehabilitation services or long-term care, can be a difficult one for our patients and their families.

Each day, lives are touched, through the efforts of our caring and compassionate professionals here at Sebos. Our staff touches these lives by providing dignified, compassionate, and individualized care to each patient that we have the privilege to serve.

As part of our short-term rehabilitation program, a significant percentage of our patients return home following a brief stay with us. For residents who are no longer able to return home, we offer long-term care services in a comfortable and home-like setting.

For family members, caring for someone with Alzheimer's disease at home can become overwhelming and often become no longer possible as the disease progresses. We understand both the emotional and physical challenges that come with Alzheimer’s disease and other dementias, and we are here to help.

Sebo’s Nursing & Rehab offers specialized care for patients with Alzheimer’s disease and other forms of dementia and memory loss.

We’re dedicated to preserving the dignity and individuality of every resident, and we develop a care plan for each resident that focuses on his or her abilities, while addressing the obstacles associated with the disease process.

The Sebo’s Nursing and Rehabilitation Center was named a Four-Star Award Center for Medicaid/Medicare Services by the U.S. Government Site for Medicare. The center’s amazing health and fire safety inspections, staffing, quality measures, and penalties all added into this four-star rank.

Melissa Cox, the administrator of Sebo’s Nursing & Rehab, said it is thanks to the team of nurses, doctors, and everyone inside the facility that helped raise Sebo’s to this level.

“We are so proud to have become a four-star building,” Cox said. “We have always prided ourselves on having a strong team that works together to make our facility a place our residents are happy to call home. Our hard work and dedication finally has given us the ability to show the community that Sebo’s is the place to be.”
