Through these Accountable Care Organization partnerships, physicians, hospitals, and other healthcare providers come together to coordinate high-quality care to Medicare patients. This ability to coordinate services creates an environment that not only aids the patient, but proves that these two facilities are able to provide the best in care for those in need.
Sebo's Nursing and Rehab, Spring Mill Health Campus, and St. James Manor were selected by Community Healthcare Partners alongside Miller's Merry Manor in Portage, Symphony, the Life Care Center of Valpo, and Hartsfield. These facilities have maintained top scores from the Centers for Medicare and Medicaid Services, and strive to provide top-notch service to their patients at all times.

St. James Manor and Villas which is located at 1251 E. Richton Road in Crete, Illinois, minutes past the Indiana border is honored to be chosen as the only Illinois facility to partner with the Community Healthcare Accountable Care Organization.
