A View Into What’s New with NorthShore Health Centers

FamilyCareNorthShore Health Centers has a new Patient Portal that they are excited to offer to their patients. This Patient Portal is an easy to use, Internet-based portal that makes communication with Providers easier.

The Patient Portal can be accessed from anywhere as long as you have an Internet connection or a smartphone with data capabilities. But why should you use the Patient Portal?

Everything you need is in one place so you can:

  • Schedule an appointment
  • Request prescription refills
  • Access your health information
  • Obtain educational information
  • Message your Provider
  • Review your latest test results
  • Request electronic copies of health records
  • View statements and pay online

All of this can be done without leaving the comfort of your own home. You can also enroll online through a safe and secure Patient Portal Account. Getting the coverage that you need has never been easier and if you are unsure about how to use the Patient Portal there are additional resources available to you like a User Guide and Patient FAQs. And there is always someone from NorthShore Health Centers available to help you.

So why the creation of the new Patient Portal? NorthShore wants to keep giving the high quality care to people in the region. And the drive and results that the organization produce is nationally recognized. Recently, NorthShore Health Centers was recognized as a Quality Improvement Award recipient and awarded Affordable Care Act funding by Health and Human Services to expand and improve the quality of care that they provide.

A little more than 1,000 organizations throughout the US received this funding. Those that did were chosen because they provided high levels of quality performance in one of four categories:

  1. Health center quality leaders – be among the top 30% that got the best overall health clinical outcomes.
  2. National quality leaders - receive awards that denote the exceeding of national clinical standards.
  3. Electronic Health Record (ERS) reporters – utilize ERS to report clinical quality measure for all of their patients and receive awards for doing so.
  4. Clinical quality improvers - received awards that denote at least a 10% improvement in clinical quality measures between the years 2012 and 2013.

NorthShore aced three of the four categories, meaning that they are demonstrating top-notch care and service and now they can do even more to make sure that all of their patients’ needs are met and will continue to be met in the future.
