NorthShore Health Center is a not-for-profit community based health and education center, with two locations in Portage, two locations in Lake Station, Merrillville, and in mid-December 2013, a new location in Hammond, Indiana. Our mission is to provide health and dental care to the under and uninsured, with a focus on serving Northern Porter and Northeast and Central Lake counties. The number of people seeking services continues to grow at a rate of between 600-650 new patients per month.
Today, we are asking for your help to meet the growing healthcare needs of Northwest Indiana residents. In return, you will receive tax credits to reduce your overall tax burden. The Indiana Housing and Community Development Authority through the Neighborhood Assistance Program (NAP), has granted tax credits to NorthShore Health Centers to cover the expenses of a Nurse Practitioner. These credits allow donors to financially support NorthShore Health Center and to take advantage of Indiana’s innovative tax credit program ensuring that your donation goes farther in providing a direct benefit to you. The following is an example of how the credit benefits you:
- A tax credit on your Indiana tax form valued at 50% of your donation.
- A tax deduction on your federal tax form of up to 100% of your donation (based on donor tax bracket).
- NorthShore Health Centers benefits at the level of your donation.
Your Donation = $1,000 – to NorthShore You receive 50% Indiana State tax = $500 – credit or refund If 28% tax bracket Federal return = $280 – credit on Federal Tax Actual cost to donor = $220
